Dinky Gonuts T-shirt 1982 Cereal Prize

Not sure where this T-shirt fits in. Is there a set of four T-shirts, or just the one? Was it found in stores or a mail-in offer? Other Wacky T-Shirts displayed entire wacky stickers, but this one is just the art without the cereal box.

The art was repainted and they added the broken end of the spoon, which is out of frame in the sticker below.
